Below are forms which should be filled out prior to the initial appointment.  Please print the forms that are relevant to you and/or your child and bring to the appointment with you.  The first sections should be completed by every new patient.  In the second section, please print the forms that seem to best address the symptoms or concerns you/your child are experiencing.

Please also complete the 'Release of Information Form' and bring to the initial appointment if you authorize us to contact any of your past treating clinicians.  The Federal Government requires you to know about your health information, so please read the Privacy Practices and sign the acknowledgement.

If this is the first evaluation your child will have please fill out the Long version of the Child History Form.  If you have previous assessments that you are able to bring to the first appointment you may fill out the Short version.
