The evaluation process is comprehensive and often requires two hours. This evaluation is completed into two one hour sessions or one longer session. Typically, the first is an intake session with Jamie Roberts Licensed MFT who discusses history and present concerns. The second appointment will be with Dr. Ott for the assessment, treatment plan, and medication options. Both sessions together are $700. Follow up appointments are 30min and $200. Dr. Ott is not a provider for any insurance networks and payment is due at the time of service. We can provide you with a superbill to submit to your insurance for possible reimbursement.
For children and adolescents, this initial process includes interviews of the parent(s) and child both separately and together. (There is a play room and a large waiting room near Dr. Ott’s office where the child can wait.) After the initial appointment is scheduled, members of Dr. Ott’s staff may contact you for confirmation and additional information. Furthermore, past records (from prior clinicians) and reports (psychological or neuropsychological testing, speech and language testing, school reports, etc.) should be brought to the initial evaluation in order for Dr. Ott to review.
Before coming in, please visit the forms page and fill out the appropriate information. Please bring these to the first visit with you. Every new patient should bring a History Form, a Patient Information form, and a Release of Information (if necessary). There are two child history forms available (long and short). If your child already has any previous evaluations or assessment, you may fill out the short form and bring the additional documentation with you. If this is the first evaluation, please fill out the long form so appropriate developmental information is included. Please email or call the office if you are unsure of which forms to fill out.
